Why aren’t the cannula holes drainage routes?

In the world of post-operative cosmetic surgery, there is a widespread but dangerous practice: attempting to «milk» or force out fluid through the incisions the surgeon made for the cannula. The Myth of the «Hole» as a Drain

Anatomically, the human body has its own drainage system: the lymphatic system. The cannula holes are incidental surgical wounds, not conduits designed for fluid transport. Forcing fluid into them goes against the body’s natural flow. Proper drainage should be proximal, directing lymph toward healthy lymph nodes (armpits, groin, etc.), not into an open wound

Mechanical Damage to Healing

When we manipulate or try to keep these incisions open, we disrupt a vital biological process.
Fibrin Destruction: The body creates a natural fibrin «plug» to close the wound. By constantly reopening it, we destroy this seal, causing healing to stall and the wound to widen.

Periorificial Fibrosis: Excessive pressure around these points creates constant trauma to the tissue, resulting in dark, sunken, and hardened scars that are very difficult to correct later.

Failed Adhesion: For the skin to «adhere» to the muscle again, it needs a stable environment. The constant forced outflow of fluid prevents the tissue layers from bonding properly.

The Risk of Using Foreign Objects

It is alarming to see how, to keep these openings open, objects such as external cannulas, cotton swabs, or even fingers are inserted without proper aseptic technique.

Beyond Cross-Contamination: Introducing foreign objects into a surgical incision not only invites external bacteria (staphylococci, streptococci) into the deeper space, but also generates thermal and mechanical trauma that can cause necrosis of the wound edges.

Entrance to Infection: A wound that is opened and closed manually becomes the perfect breeding ground for cellulitis or a subcutaneous abscess, which could lead to a hospital emergency
